Not able to send email

I have setup phplist
but not able to send email.

while sending test mail it gives following error
"Sent test mail to: XXXXX@XXXXXXX.XXX failed."

Error while sending actual campaign
"Error sending message 5 (2/10000) to XXXXX@XXXXXXX.XXX"

Can you describe how you set it up (what instructions did you follow) and what your server situation is (shared hosting? etc)
